Sterilizer monitoring is one of the most frequently cited weak points during dental board inspections — not because practices lack autoclaves, but because documentation is incomplete or testing frequency falls short of CDC recommendations. Understanding the three-tiered monitoring system, and how to document it defensibly, is essential for any office reprocessing instruments in-house.

The Three Required Monitoring Methods

According to the CDC, the ability of a sterilizer to reach conditions necessary to achieve sterilization should be monitored using a combination of mechanical, chemical, and biological indicators, with mechanical indicators assessing cycle time, temperature, and pressure found on displays and printouts of autoclaves. Chemical indicators use sensitive chemicals to assess physical conditions during the sterilization process, changing color when exposed to high temperatures or combinations of time and temperature.

Biological indicators, or spore tests, are the most accepted method for monitoring the sterilization process because they assess the sterilization process directly by killing known highly resistant microorganisms such as Geobacillus or Bacillus species. A spore test should be used at least weekly to monitor sterilizers, and should use a matching control — meaning the biological indicator and control are from the same lot number. A spore test should also be used for every load with an implantable device.

Why Layering Matters

Because spore tests are only performed periodically and the results are usually not obtained immediately, mechanical and chemical monitoring should also be performed with every load. This layered approach means a failure can be caught in real time through mechanical or chemical readings, even before spore test results return days later.

What to Do With a Failed Spore Test

The CDC recommends biological indicator tests be run at least once per week, and if the biological indicator fails, the CDC recommends running a second test. Until results are available, the sterilizer should be considered non-functional and any instruments from that batch quarantined, while mechanical/physical and chemical monitoring should be completed with every load in the meantime.

Documentation and Recordkeeping

Maintaining accurate records ensures cycle parameters have been met and establishes accountability, and if there is a problem with a sterilizer — such as an unchanged chemical indicator or a positive spore test — documentation helps determine if an instrument recall is necessary. For every load, practices should capture the date, sterilizer ID, cycle parameters, load/lot number, operator, contents, mechanical monitoring data, chemical indicator results, and biological indicator reports.

Retention periods are not federally standardized. Maintain your sterilization monitoring records — mechanical, chemical, and biological — long enough to comply with state and local regulations, since requirements vary by jurisdiction. Some state rules specify multi-year retention, so practices should confirm the exact figure with their state dental board rather than relying on a national default.

State Variation Is Real

Dental offices in the United States are required to monitor sterilization equipment using biological indicators to ensure patient safety and compliance with infection control standards, and while most states follow CDC recommendations for weekly spore testing, specific requirements and enforcement vary by state dental boards. Some boards import CDC guidance by reference, meaning the requirement automatically updates if CDC guidance changes — so periodic review of both federal and state sources is a sound compliance habit rather than a one-time task.

Building an Audit-Ready Routine

Practices that pass inspections cleanly typically share a few habits: a dedicated log (paper or digital) updated at the time of each cycle rather than after the fact, matched lot numbers between test and control biological indicators, and a written corrective-action procedure for any indicator failure. Treating monitoring as a documented system — not just a task — is what converts routine sterilization into demonstrable regulatory compliance.
