Choosing the Right Mask: ASTM Levels Explained

Not every procedure carries the same risk of fluid splash or aerosol exposure, and not every mask offers the same level of protection. Selecting the correct mask for the task is a simple but often overlooked step in maintaining both staff safety and cost efficiency. The ASTM F2100 standard provides a useful framework for matching mask performance to procedure risk.

Understanding the ASTM Levels

Face masks are typically classified into three ASTM performance levels based on fluid resistance, filtration efficiency, and breathability:

  • Level 1 (Low Barrier): Suitable for routine exams, short procedures, and general patient interactions where minimal fluid, spray, or aerosol exposure is expected.
  • Level 2 (Moderate Barrier): Designed for procedures involving light-to-moderate fluid exposure, such as basic restorative work or moderate-length surgeries.
  • Level 3 (High Barrier): Provides the highest fluid resistance and filtration, intended for procedures with heavy blood or fluid exposure, high-speed instrumentation, or aerosol-generating techniques such as crown preparation, periodontal surgery, or implant placement.

Matching Mask to Procedure

A practical rule of thumb: the greater the anticipated splash, spray, or aerosol generation, the higher the barrier level required. Procedures using ultrasonic scalers, high-speed handpieces, or air-water syringes generate significant aerosol and should default to Level 3 protection. Diagnostic exams, consultations, and suture removals typically do not require this level of barrier protection and can be safely managed with Level 1 or 2 masks.

Using a higher barrier level than necessary isn't harmful, but it does increase cost and may reduce staff comfort over long shifts due to decreased breathability. Conversely, under-protecting during high-risk procedures increases exposure risk to both clinician and assistant.

Fit and Seal Matter as Much as Level

Even a Level 3 mask offers limited protection if it isn't fitted properly. Gaps at the nose bridge or cheeks allow unfiltered air and droplets to bypass the mask entirely. Staff should be trained to:

  • Mold the nose wire snugly against the bridge of the nose
  • Ensure ear loops or ties are secure enough to prevent gapping
  • Replace masks between patients or if visibly soiled or damp
  • Avoid touching the front of the mask once positioned

Respirators for Airborne Precautions

When airborne precautions are indicated—such as for suspected tuberculosis or during specific aerosol-generating procedures on patients with respiratory infections—a fit-tested N95 respirator is required rather than a standard surgical mask, regardless of ASTM level. Surgical masks are not designed to filter airborne particles at the same efficiency as respirators.

Building a Practical Mask Protocol

Practices benefit from stocking at least two mask levels: a Level 1 or 2 mask for routine daily use, and a Level 3 mask reserved for surgical and aerosol-heavy procedures. Keeping masks organized by level near point-of-use areas—rather than in a single mixed supply bin—reduces the chance of staff grabbing the wrong protection level during a busy schedule.

Reviewing mask selection periodically alongside your procedure list ensures your protocol keeps pace with any changes in services offered, from added surgical capabilities to new aerosol-generating equipment.
